Participants in the AltiusExtra program acknowledge that there is a possibility that contracted vendors providing goods and/or services in connection with the program can cause injury and damage to a participant. The participant understands and accepts such risk. Altius Health Plans and its program contractor, Basix, disclaims any warranty, expressed or implied, regarding the quality, nature or kind of goods and/or services to be rendered to a participant. Altius Health Plans and/or Basix shall not be responsible or liable to the Participant under any circumstances for any injury or damage sustained by the Participant as a result of goods and/or services provided under the program. Altius Health Plans and/or Basix shall not be liable for any damages, including without limitation, direct, indirect, consequential, incidental, special or exemplary damages arising out of or related to the provision of goods and/or services to the Participant.
